Step 1: Complete and Return Waiver
Please fill in, sign, and return the Finish Strong Waiver. This is required for participation. Please return the completed form to: membership@teamfs.org.
Returning members who have completed a waiver do not need to complete this step.
Note: In order to fill in and electronically sign the waiver, you must have Adobe Acrobat Reader DC installed on your PC or a current version of Adobe Acrobat Reader installed on mobile devices.

Step 2: Sign and Return Competitor’s Creed
As followers of Christ and members of Team Finish Strong, we adhere to the Fellowship of Christian Athlete’s Competitor’s Creed. Please read, sign and return the FCA Competitor’s Creed form and return to: membership@teamfs.org.
Returning members who have completed the FCA Creed do not need to complete this step.

Step 3: Remit Annual Membership Fee
Memberships are processed by our payment provider: Square. Click on Membership Payment Processing for online payments. For offline membership processing, please contact membership@teamfs.org for additional payment options.

Step 4: Submit Membership Application
By submitting the below information. I am certifying that I am 18 years old or older. Please remember to complete your waiver and remit your membership fee at the time your application is submitted. Applications submitted without waivers or membership fees will not be processed.
